F2F Family Navigation Specialist Job Description

Reports to: F2F Program Coordinator, Associate Director 

Position Summary: Provide navigation support for parents who reside in the Olympic and Kitsap Peninsulas of Washington state and experience an IDD and/or are neurodiverse, that are involved with family support service agencies and organizations and/or the Parents Representation Program, if the parent is being or has a potential to be referred to CPS for out of home placement.  

Job Responsibilities: 

  • Identify parents residing in Clallam and Jefferson counties as well as parts of Grays Harbor, Mason, and Kitsap counties, who experience an IDD and/or are neurodiverse, that are involved with family support service agencies and organizations.
  • Provide support, training, information, and resources to parents who experience an IDD and/or are neurodiverse, especially if the parents have children who experience complex medical conditions and/or experience an IDD.
  • Work collaboratively to assist and modify a community services Roadmap in support of parents with IDD/DD and/or who are neurodiverse.
  • Connect parents with community partners, as needed, including PAVE’s Helpline, and using its proprietary database, ConnectedFamilies.
  • Collaborate with service providers for additional resources and information for parents.
  • Utilize customer service surveys to evaluate the navigation support provided and to incorporate parent feedback for continuous improvement of services.
  • Work with the Program Coordinator to set up an outreach plan for case managers, DDA offices, CPS offices and hospital to share the program for referral points. (County P2P, CYSHCN nurses, B to 3, Home and Community based waver services, etc.) 
  • Ensures appropriate referrals are made both internally and externally and are included in the roadmap. 
  • Using PAVE’s proprietary ConnectedFamilies database to enter, update, and share information sent, shared, and/or provided to parents and professionals.
  • Work with Program Coordinator to develop bridging strategies to help teams understand the roadmap process as a living document and communication tool within the team and with the family/parent. 
  • Work with the F2F team and the data and curriculum team to develop and implement an evaluation Q/I process. 
  • Attends outside networking and referral source meetings as requested by supervisor. 

Job Requirements for all Staff:  

  • Maintain confidentiality of families and other staff members.
  • Be prompt in arriving at any, in person or virtual meetings, and workshops if required.  
  • Submitting accurate time sheets daily is preferred and at least weekly to include description of work performed.  
  • Attend and participate in staff meetings as scheduled.  
  • Assure your schedule is up to date on the Shared Calendar  
  • Assist all colleagues in meeting timelines.
