What is Find Your Facilitator?
PAVE in partnership with the Dan Thompson Account created this searchable, HIPAA-compliant secure online database. The free tool is accessible, and we hope will offer many options for all individuals with a disability, especially for DDCS clients and their loved ones – families, spouses, friends, neighbors, teachers, etc. – across WA. PAVE gratefully acknowledges all who assisted to offer this statewide resource.
Who can be included in Find Your Facilitator?
Person-Centered Facilitators providing services in-person and/or virtually in WA that has the following:
- Must have completed a PCP Facilitator Training
- Must be available to facilitate planning sessions
- Must have completed a DSHS background check
