Family to Family Health & Information Center (F2FHIC)
F2FHIC helps families of children and youth with special health care needs, and the professionals who serve them, to find healthcare funding. The Family-to-Family Health Information Center provide sadvice, offers a multitude of resources, and taps into a network of other families and professionals for support and information.
Person Centered Planning Program (PCP), is more than a process it’s a way of thinking and supporting people. It values each person’s strengths, interests, and dreams. Our role is not to decide for them, but to listen and help shape the life they envision. PAVE offers virtual, on-demand (go at your pace) PCP Facilitator Training and Find Your Facilitator, a statewide database of Person-Centered Plan Facilitators.
